PediaSure Complete Survey Contest Rules

PRIZES

One (1) prize is available to be won in the Contest (“Contest”): One (1) Prize of an iPad valued at $800 to the winner. This prize will be awarded to the person whose contact information (name and/or telephone number or email address) has been registered on the selected qualifying entry. The prize will be awarded within 4 to 6 weeks of the prize draw. There will be no regional allocation of prizes. The odds of winning depend on the total number of qualifying entries received.

All prize claims are subject to verification by the Contest organizers and/or their designated representatives. Abbott will be the sole and final reference in validating entry forms and prize claims.

HOW TO ENTER

Entry Forms. Eligible persons who complete a web survey during the Contest period @ www.pediasurequestions.ca are automatically entered in the Contest for one (1) chance to win the prize. The Contest runs from 12:00 a.m., Eastern Time, September 12th, 2011 to 11:59 p.m., Eastern Time, December 21st, 2011. All surveys and entries become the property of Abbott.

Eligibility. No purchase is necessary to enter the Contest. Entrants must be Canadian residents who have attained the age of 18 years on the date of entry in the Contest. Ineligible entries will be disqualified. Abbott shall not be responsible for any Contest documentation that is illegible, incomplete, lost, stolen, or not received for any reason whatsoever.

Restrictions. Each person, household, group or organization is restricted to one (1) entry for the Contest. Entrants leaving a common telephone contact number will be considered, for purposes of the Contest draw, to belong to the same household, group or organization. Any excess survey completions will be deleted for purposes of the draw. Illegible, incomplete or incorrectly submitted entries will be disqualified. Employees of Abbott, the Contest organizer, or their affiliates, their immediate family members and those sharing the same household as those individuals are ineligible for the Contest.

SELECTION PROCESS

There will be one (1) draw for the prize on December 22nd, at 2:00 p.m., Eastern Time from all of the eligible entries received, held at the offices of the Contest organizer. The selected qualifying entrant will be contacted by phone or email at least three (3) times within the fourteen (14) days after the draw. Where a selected entrant cannot be contacted because of incomplete or incorrect information, Abbott will use reasonable efforts to contact the selected entrant. If the entrant cannot be contacted in that time, this entry will be disqualified and another entrant will be drawn from the remaining eligible entries.

 

Once the selected entrant is notified, to be declared a Contest winner, the selected entrant must first correctly answer a time-limited, mathematical, skill-testing question to be administered by telephone, and he or she will be required to sign a standard declaration and release form confirming compliance with the Contest rules, acceptance of a prize as awarded and releasing Abbott and the Contest organizers from all liability. Each selected qualifying entrant will have seven (7) days from receipt to return the declaration and release form. Upon receipt of the declaration of release form within this period, the selected qualifying entrant will be declared Contest winner. If the declaration of release is not received within this period, the selected entrant is disqualified and another eligible entry will be drawn. It is possible to obtain the name of the Contest winner by sending a stamped, self-addressed envelope to PediaSure® Complete Survey Contest at 130 Chemin des Renards, Rigaud, Quebec, J0P 1P0 prior to January 31st, 2012.

GENERAL

Neither Abbott, PediaSure® Complete retailers, or any organization involved in organizing the Contest shall be responsible for any Contest documentation that is illegible, incomplete, lost, stolen, misdirected, not received, destroyed, or delayed for any reason whatsoever.

By entering, Contest entrants agree to abide by the Contest rules and regulations and the decisions of Abbott and the Contest organizers,which are final. Abbott reserves the right to change or alter any of the Contest rules and regulations or to cancel the Contest at any time. The prize must be accepted as awarded and is non-transferable.

By entering, Contest entrants agree that Abbott is not responsible for any technical errors, inaccessibility of the website, inaccuracy of the information provided by Contest entrants and damages to Contest entrants’ computers.

By accepting a prize, the winner agrees to the use of their name, address and/or photograph, without payment, in any subsequent publicity or advertising carried out by Abbott and/or its advertising agents.

By submitting any information whatsoever, Contest entrants agree that the personal information provided to enter the Contest will be collected, used, stored and/or disclosed only to administer the Contest, and for no other reason. Once the Contest is over, Abbott will destroy the personal information.

For Quebec residents, any litigation respecting the conduct ororganization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the board only for the purpose of helping the parties reach a settlement.
